About CatholicCare NT and the Program

CatholicCare NT is a Catholic social service agency established in the Diocese of Darwin. We provide services throughout the NT, including service centres in Darwin, Palmerston, Malak, Tiwi Islands, Daly River, Wadeye, Jabiru, Maningrida, Katherine, Tennant Creek, Alice Springs, Santa Teresa, Titjikala, Finke and APY lands.

Our Housing Support Program implements a case management approach to break the cycle of homelessness through empowering clients to effectively problem solve and manage their accommodation. CCNT uses an early intervention approach that supports individual clients to regain control of their circumstances including; housing, financial, social and community issues that are placing them at risk of homelessness.

Through assertive case management we will provide clients with support to address the multifaceted challenges they face in maintaining their home. This support may be through assessment of life skills, role modelling, referral to specialist agencies as required, coordination of supports, and advocacy with internal and external agencies. CCNT’s case management framework adheres to the Standards of Practice established by the Case Management Society of Australia and New Zealand and Affiliates.

Youth Outreach Services aim to assist young people in Tennant Creek who are at risk of homelessness to stabilise their personal circumstances. We achieve this by engaging them in intensive support, case management and other related service sectors to respond effectively to young people who have complex needs and are homeless or at risk of homelessness. The service supports vulnerable young people (12-18 years) to re-engage with their families and community, and address issues impacting upon their social and emotional wellbeing, which may lead to behaviour that is harmful to self and or others.
