Key Responsibilities:
- Encourage and promote client independence, wellness and reablement.
- Provide high-quality advocacy assistance to people with disabilities through the joint development of a client advocacy action plan and provide information and advice, establish referral pathways and assist people with disabilities, their families, carers, and local support networks.
- Keeping a clear communication with the family members of the clients and support workers to follow up the improvements and achievements of their goals, with monthly home visits.
- Educate and explain the process of filling intake assessment forms, service agreements and invoicing.
- Educate eligible members of society about the NDIS program and offer them with the resources they need to lodge a funding request.
- Assisting participants to understand and budget their plan.
- Assist the participants to overcome any hurdles or barriers they may have to employment including assistance with obtaining required training, licences, meeting industry demands and confidence building.
- Liaise with support coordinator to acquire resources for the planning of diversional-therapy activities.
KEY SELECTION CRITERIA
- Diploma of Community Services or Disability Services, or Mental Health
- Awareness and understanding of the NDIS and home care support programs.
- Excellent written and verbal communication, time management and organizational skills
- Ability to advocate, negotiate and problem solving
- Current Working with Children check
- NDIS Worker Screening Check
