Ethnic Community Services Cooperative (ECSC) has been supporting multicultural communities since 1979. We are a leader in the provision of bicultural and bilingual support for individuals and communities and have been recognised internationally through UNESCO.

We provide CHSP services for CALD elderly, disability support coordination and community access as a registered NDIS provider and playgroups and preschool support for multicultural children and families.

ABOUT THE OPPORTUNITY

ECSC is seeking a Support Coordinator to work together with participants on the National Disability Insurance Scheme (NDIS), their families and carers to provide end to end coordination of services and supports. You will work with our participants and be based at our Marrickville office.

KEY RESPONSIBILITIES:

Key Responsibilities include but not limited to:

  • Providing Support Coordination Services to NDIS participants.
  • Support participants to understand their NDIS plan and navigate the NDIS portal.
  • Think creatively to achieve participants' goals.
  • Work with internal and external stakeholders to achieve positive outcomes for participants
  • Act promptly on referrals, ensuring timely support.
  • Assess and select appropriate providers with experience in dealing with complex issues.
  • Negotiate service to be provided and create service bookings.
  • Determine the budget for each support type and advise the relevant plan manager.
  • Link to mainstream or community services as identified.
  • Support participants to understand their responsibilities under service agreements with ECSC.
  • Assist participants in preparing for plan reviews, evaluating progress towards goals, and identifying suitable options to addressing challenges.
  • Prepare Service Agreements and NDIA consent forms.

SKILLS & EXPERIENCE (Selection Criteria)

  • Experience implementing person-centred plans and providing support to people with a disability, their families, and carers
  • Demonstrated experience using Client Management Systems to maintain accurate records
  • Fluency in Vietnamese will be highly regarded
  • The ability to work with people from diverse backgrounds, and age ranges.
  • Good communication skills (written and spoken).
  • Good understanding and use of technology.
  • Able to resolve conflict
  • Able to understand the NDIS Price Guide and flexibility within budgets, the NDIS legislation and rules including provisions relating to reasonable and necessary supports and the role of the mainstream service system
