  • Contract: Fixed-Term Contract until 30th June 2025 – potential extension.
  • Hours: Full time – Monday – Friday, 12:00pm – 20:00pm.
  • Hours: Part-time – Minimum of 24 hours a week across 3 days – 16:00pm – 20:00pm.
  • Flexible working – hybrid ways of working with a minimum 2 days per week in the office for Full Time and 1 day per week for Part Time.

Day-to-Day Duties:

As part of SANE Australia, Telehealth and Online Mental Health Recovery Planner plays a key role in understanding the individual needs of participants, facilitating the development of personalised recovery plans, and providing recovery-oriented case-management to priority participants.

The Telehealth and Online Mental Health Recovery Planner provides support that is trauma informed, person centred, recovery oriented, and culturally competent. This role will work within a team of Recovery Planners to undertake comprehensive risk and needs assessments, use, and interpret recovery measures, co-design recovery and safety plans, allocate and support transitions within SANE programs, and provide recovery supports for complex and priority participants.

  • Work with people with complex mental health issues and their supporters, families, and carers to undertake comprehensive assessments, utilising organisational risk and safety screening tools.
  • Work collaboratively as a team, to develop individual mental health recovery plans for all participants entering the program.
  • Support participant progression through Guided Service, ensuring a smooth transition to appropriate intervention and specialist support teams and group programs, identified through their recovery plans.
  • Hold a small, protected caseload of participants with highly complex presentations, providing case-management and individual multi-session supports using evidence-informed approaches within your scope of practice.
  • Conduct thorough suicide risk assessments and safety planning for participants in line with SANE procedures and model of care.
  • Support continuous quality improvement processes by actively contributing to the development of your team procedures, guides, practices and ways of working.
  • Review and interpret participant survey data to provide meaningful outcomes for participants throughout their journey.
  • Ensure professional standards are upheld including privacy, confidentiality, and duty of care as well as those relevant to professional registration requirements and scope of practice as applicable.
  • Appropriately document contacts with service users utilising Salesforce, Microsoft Office, and SharePoint to ensure SANE Clinical Governance and Data Collection Policies are adhered to.
  • Contribute to research and evaluation projects and the development of content across a range of mental health issues.
  • Engage in self-led wellbeing planning to support ongoing compassionate practice and professional resilience.

Experience/Qualifications:

  • Registration or eligibility to register prior to commencement with one of the following professional bodies: AASW/AMHSW – Full membership, AHPRA as a registered occupational therapist, nurse, psychologist, PACFA – minimum provisional working toward full registration, ACA - minimum level 2.
  • Tertiary qualification in social work, occupational therapy, psychology, counselling, nursing.
  • A minimum of three years relevant mental health work experience post qualification.
  • Experience using Salesforce.
  • Current National Police and Working with children check.

Skills & Expertise

  • Highly developed skills in complex assessment and recovery planning, clinical reasoning and goal setting, problem solving, organisational and prioritisation skills.
  • Proven ability to understand and integrate the principles of case management.
  • Sound knowledge of peer work principles and practices.
  • Demonstrated commitment to trauma informed, recovery oriented, person centred, and culturally competent service provision.
  • Demonstrated high level communication and interpersonal skills.
  • Highly developed computer skills and digital service provision skills.
  • Advanced qualifications in psychological therapies, alcohol and other drugs, family & domestic violence, suicide, chronic self-harm, personality disorders.
  • Experience in participant outcome measurement tools.
  • Experience integrating peer support work into care services.
  • Experience delivering digital mental health support post-qualification.
