About the role

The Integrated Complex Care service, delivered in partnership with the Exceptional Needs Unit, aims to support individuals with mental ill health within a framework of recovery that is focussed on strengths and capabilities. The service will treat clients with compassion, provide specialist advocacy and trauma informed care and work with existing services to ensure a coordinated, integrated service response to goals identified in a recovery plan. Clients are supported to build capacity and independent living skills, establish connections with employment and educational services, create linkages with housing supports, enhance relationships with families and carers, participate in social activities and develop community networks.

We are seeking a compassionate, self-motivated Community Mental Health for a permanent, full time opportunity to work across the Adelaide Metropolitan area. You will provide services to clients, families and carers in line with Mind’s Model of Recovery Oriented Practice, My Better Life model for people with psychosocial disability. In this role you will deliver personalised support services and recovery-oriented care to people living with mental ill health in the community.

You will play an integral role working collaboratively with clients to build daily living skills, strengthen relationships with families and carers, enhance connections with employment and community networks, achieve wellbeing goals and develop capacity to live independently.

Key responsibilities

  • Deliver high quality services and facilitate positive educational and social experiences.
  • Support clients to return to their community, live independently and enjoy a meaningful life through a staged approach to recovery.
  • Provide information and advocacy to clients to establish connections with families and carers, create community networks and access to relevant services.
  • Facilitate, as appropriate, the re-engagement and maintenance of family and carer relationships.
  • Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to client’s recovery goals.

What you’ll bring

  • Tertiary qualifications (minimum Certificate IV) in Mental Health, Peer Work, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind.
  • Proven experience in Community Services, Mental Health, Disability, Social Welfare, Housing, Healthcare or Government sectors is required.
  • Experience and expertise in working directly with people with mental health issues, complex needs and with their families and carers.
  • Demonstrated understanding of available community services, networks and supports.
  • Experience in assessing need and working collaboratively to plan goal focused recovery using evidence informed approaches and tools.
