About the Opportunity

At Uniting, our Case Managers are called Community Support Partners. This role is responsible for the case management of clients who have complex care needs, making a difference in the delivery of quality and collaborative Home and Community Care services. The Community Support Partner will utilise specialist strategies that are person-centered, to produce outcomes that enable clients to effectively participate in decision-making related to their care, and access services, and to keep them engaged in their care journey. 

This is a permanent full-time role and is based at our office in Port Macquarie. The role will require travel across the mid north coast region to meet the needs of our clients.

About you

  • Tertiary or Diploma qualification in a relevant field, such as Social Work, Psychology, Aged Care, Health and/or working towards or equivalent years of experience
  • Current Australian Health Professional Registration Authority (AHPRA) and/or Case Management Society of Australia and NZ certificate, in line with qualification/s
  • Experience in the Aged Care, Community Services and/or the Health environment
  • Significant understanding of client needs, to convert these into positive service delivery outcomes
  • Ability to effectively plan and manage budgets
  • Confident in building and maintaining strong working relationships with internal and external stakeholders
  • Passion for social change and contributing to an organization of influence for vulnerable, at risk and disadvantaged members of the community
  • Proficient in using multiple computer applications, as well as the willingness to learn new systems/programs
  • First Aid Certificate
  • Current driver's license and access to a comprehensively insured vehicle
