This important and exciting opportunity builds on previous palliative and end of life models of care work to develop and implement new and innovative palliative care projects or expand on existing activities, in order to improve consumer experience and access. The Project Coordinator, Palliative Care is responsible for this vital work in SNSW.
<h3>About the Role</h3>
Drive awareness of palliative care and facilitate improved access to, and coordination of, safe quality integrated palliative and end of life care services and ensure patients/carers palliative care choices are met. These roles will work with local stakeholders to develop and implement person centered initiatives that:
<ul>
<li>Improve access to palliative care at home and support end of life care systems and services in primary health care and community care,</li>
<li>Enable the right care, at the right time, in the right place to reduce unnecessary hospitalisation,</li>
<li>Generate and use data to support continuous improvement of services across sectors,</li>
<li>Use available technologies to support flexible and responsive palliative care at home, including after hours.</li>
</ul>
<h3>About you</h3>
<ul>
<li>Relevant tertiary qualifications and experience in aged care, end of life care, or primary/community health settings,</li>
<li>Demonstrated ability to engage with and work collaboratively with a wide range of internal and external stakeholders to achieve innovative and practical solutions,</li>
<li>Ability to work unsupervised, under pressure, prioritise commitments and meet critical deadlines while maintaining quality standards,</li>
<li>Demonstrated skills and experience in delivering projects in complex and specialised environments,</li>
<li>An affinity with the purpose and the values of the organisation.</li>

As Lived Experience Coordinator, Suicide Prevention Illawarra Shoalhaven, you will join the Suicide Prevention Team and be responsible for working with, supporting and developing the capability of lived experience members of the Illawarra Shoalhaven Suicide Prevention Collaborative (<a href="http://www.suicidepreventioncollaborative.org.au/" target="_blank" rel="noopener">www.suicidepreventioncollaborative.org.au</a>). You will also assist with Collaborative projects, ensuring lived experience and suicide prevention peer workforce priorities are considered.
To be successful in this role you will:

<li>Possess highly developed interpersonal, communication, and relationship management skills</li>
<li>Be experienced in contributing insights drawn from your personal lived experience of suicide</li>
<li>Bring demonstrated experience and results from providing support, mentoring, or coaching to vulnerable, stigmatised, or disempowered individuals</li>
<li>Be solutions- and strengths-focused</li>
<li>Have a strong sense of personal resilience and a passion for suicide prevention</li>

This position will be responsible for creating content across a variety of channels including social media, website, newsletter, and mass communications in alignment with the project&rsquo;s communication plan. It will also oversee various communications platforms including Bang the Table, Workplace and the intranet, and support the coordination of mass communication campaign such as after hours and the low intensity mental health.
<h3>About you:</h3>
We are seeking a highly motivated individual, with at least 4-5 years&rsquo; experience including demonstrated skills in developing and implementing effective communication activities across diverse audiences using a variety of appropriate channels.
You will have exceptional written and verbal communication skills, including previous experience developing presentations, media releases, case studies, website and social media content, and reports.
Additionally, you will be well organised, have great attention to detail and an ability to work under tight deadlines, be proactive, and a team player.
