<h3>About the role</h3>
<p>We are expanding our St Mary&rsquo;s House of Welcome Homlessness team and seeking experienced community support workers to join the. The Community Support Workers provide non-clinical community-based support and build respectful and compassionate relationships with participants. Our Homelessness team work collaboratively to provide a safe and supportive environment for service users, volunteers and staff. They actively engage in risk assessments and provide interim / short term case management, case coordination and referrals specific to the individual need of the client and group programs.</p>
<p>You will:</p>
<ul>
<li>Provide high quality support to participants both as individuals and in small group settings</li>
</ul>
<ul>
<li>Assess client&rsquo;s wellbeing, mental health, physical health, recovery and opportunities</li>
<li>Support clients to maintain and connect with family, friends, peers, community, and other services and supports</li>
<li>Maintain accurate and up to date documentation in the Specialist Housing Information Platform.</li>
</ul>
<h3>Key requirements</h3>
<ul>
<li>A values-based approach to work aligned with our mission</li>
<li>Previous experience in a direct service delivery role within mental health, homelessness and/or the wider community sector, with experience working in the sector</li>
<li>Skills and experience to develop support plans (and strategies) which assist participants to achieve goals that support participants with recovery and improved health outcomes</li>
<li>Experience working with people with complex needs and engaging vulnerable clients whilst maintaining clear boundaries and accountability</li>
<li>Excellent communication (verbal and written) skills</li>
</ul>
<ul>
<li>Clear and Current National Police, Working with Children and current valid Driver's license</li>
<li>Strong Microsoft Office skills and capacity to use case management software Specialist Housing Information Platform</li>
</ul>
<ul>
<li>A strong service orientation and a willingness to pitch in and help others</li>
</ul>
<p>Relevant tertiary qualifications (Certificate IV or above).</p>

<h3>About the role</h3>
<p>We are seeking two experienced community support workers to become part of the St Mary&rsquo;s House of Welcome team. These positions will specifically target support for vulnerable members in our service. One position will specifically target support for vulnerable members of the Vietnamese community (fluency in written and spoken Vietnamese and understanding of specific cultural sensitivities is essential for these positions).</p>
<p>The Community Support Workers provide community-based support and build respectful and compassionate relationships with participants. You will follow NDIS support plans to build resilience, confidence and independent living skills including (but not limited to) assisting participants to appointments, provision of transport, enhancements of daily living skills and developing capacity building strategies to increase skills, achieve personal goals and build independence.</p>
<p>The Community Support Workers provide non-clinical community-based support and build respectful and compassionate relationships with participants. You will follow the participants NDIS support plans to build resilience, confidence and independent living skills including (but not limited to) assisting participants to appointments, provision of transport, enhancements of daily living skills and developing capacity building strategies to increase skills, achieve personal goals and build independence.</p>
<h3>You will:</h3>
<ul>
<li>Provide high quality support to participants both as individuals and in small group settings</li>

<li>Adhere to service agreement requirements of the funded package and meeting of deadlines</li>
<li>Have accountability for effective utilisation of billable hours for all clients allocated</li>
<li>Maintain accurate and up to date documentation in SupportAbility</li>
</ul>
<h3>Key requirements</h3>
<ul>
<li>A values-based approach to work aligned with our mission</li>
<li>Previous experience in a direct service delivery role within mental health, homelessness and/or the wider community sector, with experience working in the NDIS sector and sound case management practices</li>
<li>Skills and experience to develop support plans (and strategies) which assist participants to achieve goals in their NDIS plans and that support participants with recovery and improved health outcomes</li>
<li><strong>Bilingual in English/Vietnamese is essential for one of these roles. </strong></li>
<li>Experience working with people with complex needs and engaging vulnerable clients whilst maintaining clear boundaries and accountability</li>
<li>Excellent communication (verbal and written) skills</li>
<li>Clear and Current National Police, Working with Children and Disability checks</li>
<li>A current valid Driver's License and the ability to undertake travel for the role claiming mileage for personal vehicle use</li>
<li>Strong Microsoft Office skills and capacity to use case management software (SupportAbility)</li>
<li>A strong service orientation and a willingness to pitch in and help others</li>
