The purpose of this position is to support the implementation of Dementia Australia’s new 1-1 peer support program for people living with dementia and their carers.

This position will actively contribute to the development of a service sector that is responsive to the needs of people living with dementia and their carers, while improving outcomes of people living with dementia and their carers.

Reporting directly to the Peer Support Program Coordinator, you will also be responsible for:

  • Build collaborative relationships with people living with dementia and their carers
  • Conduct participant screening and registration into the Dementia Australia Peer Network
  • Use peer-to-peer match policy to provide suitable 1:1 connection between Peer Leader and Peer Participants
  • Ensure on-going monitoring of participation progress, including each peer match progression
  • Provide 1:1 opportunity for Peer Leaders to debrief on the progress of peer-to-peer calls
  • Participate in and contribute to service evaluations, planning, development and innovation

To be successful in this role you will be a team player, have excellent interpersonal communication skills and meet the key selection criteria outlined below.

Key Selection Criteria:

  1. An ability to engage and work in partnership with people living with dementia, and their carers
  2. Knowledge and understanding of the impacts of dementia for individuals, carers and their support networks or the capacity to develop that knowledge base and understanding
  3. Excellent verbal, interpersonal and communication skills, including the use of active listening, unconditional regard and reflective questioning
  4. Well-developed case notes and record keeping skills within the context of a client database
  5. Demonstrated capacity to communicate and collaborate effectively within a team
  6. Proficiency in using MS Office applications and skills in data entry
  7. Strong organisational skills and ability to prioritise workload

Qualifications:

  • A relevant qualification in a health-related field and/or significant experience working with clients in the delivery of community-based services
