Able2 is a not for profit, locally focussed, disability service based in the Blue Mountains, supporting clients from Western Sydney and Penrith through through to Lithgow. Our programs are designed to improve the lives of some of the community’s most vulnerable people.

Working with children, young people and adults with a disability, our growing support coordination team supports participants to fully maximise the funding supports in their NDIS plan, with an aim to build capacity and resilience in our participants, whilst being responsive to additional needs that may arise from complex life circumstances.

You will be joining a collaborative, supportive and high performing team, where people are always willing to pitch in and help one another to ensure high quality service outcomes for participants.

THE ROLE

Enjoy an exciting and rewarding position in a supportive team where you will develop meaningful, ongoing relationships with NDIS participants and their families.

More specifically, some of your key duties will include (but will not be limited to):

  • Manage a caseload of participants, implementing the participants NDIS plan using a person centred approach.
  • Provide mentoring, coaching, capacity building support, and supervision to Support Coordinators.
  • Assist participants to connect and engage with, service providers, formal and informal supports that will assist them achieve their goals
  • Build and sustain relationships with external service providers and mainstream supports for the purpose of increasing opportunities for participants.
  • Building capacity in participants and or their parent/carer to build confidence and engage with their community

ABOUT YOU

To be successful in this role you will be a driven and compassionate professional with strong verbal and written communication skills. You will have a genuine passion in supporting people with a disability and their families in our local community and to be part of a close knit team.

You will require the following qualifications, skills and experience:

  • A relevant qualification and/or experience in case management/support coordination, or other relevant areas of the disability or community sectors
  • Management or team leader experience
  • Highly developed interpersonal skills, with the ability to build relationships with participants and external stakeholders
  • Strong time management skills, and the capacity to manage competing priorities
  • Demonstrated knowledge, experience and understanding of the support needs of people with disability, particularly those with complex support needs, whose formal support is often a combination of funded NDIS support with other mainstream interfaces
  • Experience working with people living with a disability who may require additional support with their communication or cultural needs
  • Current NSW Drivers’ License and reliable car
  • Excellent verbal and written communication skills
  • Current NSW Working with Children Check (or willingness to obtain prior to employment) and police check.
