About the Role

The role will support General Practice to provide comprehensive health assessments, facilitate in the ongoing health management, and contribute to the smooth transition to the Australian Health care system for refugees and migrants who are newly arrived in the area. Providing 3 and 6 month reviews, and exit interviews to track and assess outcomes of clients plus provide health skills education and health expertise by working collaboratively across relevant Access programs

The key responsibilities include;

  • Assess and develop care plans for health and social need as appropriate and in line with funding body guidelines
  • Facilitate referrals and coordinate health appointments to relevant health services, including but not limited to Tuberculosis screening follow-up, schedule immunisation catch ups, pathology screening, childhood development
  • Actively engage in and work collaboratively with relevant Networks/Collaboratives
  • Achieve results for settling the clients through health networking, mapping the service system, and creating and sustaining partnerships
  • Work within relevant GP's to:
  • Deliver nursing assessments
  • Improve culturally responsive practice
  • Improve the target groups experience and engagement with health services
  • Provide education to practice staff to improve culturally safe practice
  • Facilitate immunisation schedules, providing immunisations where relevant
  • Act as a client advocate and protect the rights of clients in relation to health care
  • Produce and collate statistical reports, monthly reports, quarterly progress reports and annual reports as requested
  • Plan and prioritise own work requirements effectively to meet defined patient/client care objectives within agreed timeframes

You will be someone who has proven expertise in delivering a high level of individualised client centered support with refugee & asylum seeker backgrounds. Ensure compliance, contractually and within the scope of professional guidelines in health. Your communication skills will be finely tuned and adaptable, with the ability to advocate, educate and ensure the clients receive the care and services they need for optimum health, sometimes in conflicting and complex situations.
