AMSANT is the peak body for Aboriginal Community Controlled Health Services in the Northern Territory and advocates for equity in health, focusing on supporting the provision of high quality comprehensive primary health care services for Aboriginal communities

Role Description

The Integrated Team Care (ITC) program is a national program that aims to improve the health of Aboriginal and Torres Strait Islanders living with chronic conditions. The Indigenous Health Project Officer (IHPO) position will support the NT Integrated Team Care workforce and other chronic conditions workers employed within Aboriginal community-controlled health services (ACCHS). The IHPO roles will be based at the AMSANT Darwin office and at the Alice Springs office (one role in each location) within the Workforce & Leadership Support (WALS) team.

The IHPO will work with ACCHS and other key stakeholders to actively support opportunities to enhance the improvement of chronic conditions care coordination in Aboriginal health. In particular this role works closely with the funding body NTPHN. The IHPO will also contribute to broader workforce initiatives designed to increase Aboriginal training and employment and contribute to a range of chronic condition support tasks.

Key Responsibilities:

  • Provide support to AMSANT Member Services ACCHS and their chronic conditions workforce via site visits, network development opportunities, and provision of relevant information, advice and resources
  • Plan, coordinate and host the annual care coordination 2 day workshop and regional events
  • Develop and maintain collaborative relationships with relevant stakeholders, including but not limited to AMSANT member services, NTPHN, government and non-government health and community services, registered training organisations and peak bodies
  • Engage with and assist ACCHS in the recruitment, support and work planning of their chronic conditions workforce
  • Establish relationships with Territory and National working groups with a chronic conditions focus, to identify and contribute to emerging policy and program development
  • Support the development and ongoing monitoring, implementation and evaluation of the NT ITC Workforce Support and Capacity Development Activity plan. This will include ongoing assessment of NT ITC workforce needs, and service planning in response to this
  • Provide succinct and timely reports on all aspects of the work role as required by AMSANT and the funding body
  • Monitor the program budget and expenditure, working with the line manager and AMSANT finance team to ensure the funds are expended in line with the budget
  • Contribute as an active member of the AMSANT team through participation at all relevant AMSANT meetings and forums

SELECTION CRITERIA Essential:

  1. Relevant tertiary qualifications and experience in a health related discipline
  2. Sound knowledge and understanding of chronic conditions in Aboriginal health, including a sound understanding of the social determinants of health and how these affect chronic conditions management
  3. Sound knowledge and understanding of the health and community sector, both Government and non-Government in the Northern Territory and nationally
  4. Strong oral and written communication skills and the ability to liaise effectively with a wide range of internal and external partners and stakeholders in an environment requiring cultural sensitivity
  5. Qualifications, skills or experience in training and assessment, the tertiary education 
  6. Ability to communicate and work effectively with Aboriginal people and communities
  7. Ability to analyse, research and problem-solve to support effective ITC program development, and policy development
  8. Ability to work independently, and as a member of a team, with strong organisational
  9. skills and demonstrated ability to plan, manage multiple projects and prioritise in a busy work environment

Desirable:

  1. Previous experience working in Aboriginal primary health care in the NT
  2. Awareness of the history and role of Aboriginal community-controlled health organisations and an understanding of the Aboriginal primary health care system in the NT
  3. Project or program management qualifications
  4. Clinical qualifications or experience is highly desirable
