We are looking for someone like you, who shares our vision and values, and wants to join our community as the newest Case Manager, Permanency Support Program - Foster Care & Kinship Care with our Out of Home Care Services.

About the Role

The Case Manager is responsible for all aspects of case management responsibilities and to ensure the safety, wellbeing, and positive permanent outcomes for young people within the Permanency Support Program (Out of Home Care) service.

  • Work in culturally competent ways with Aboriginal families and within Aboriginal communities
  • Develop relationships and network with Aboriginal service providers
  • Follow case management principles and procedures to work in collaboration with children, young people, their families, and their Care Team to continually assess, develop, implement, monitor, and review the planning for the child or young person.

You'll be drawing upon your experience and knowledge of the child and family sector on a daily basis in this role, as well as your experience in working alongside Aboriginal communities, so those top-level communication and culturally competent skills will be needed. You will also need a minimum diploma qualification in Community Services or a related discipline.

You will be someone who is able to work autonomously at times, respecting on the trusting relationship you have with both our service and the families and children you will be supporting.
