We are seeking someone qualified and experienced to work within our residential services team based at our Dandenong office. The Case Manager directly supports young people who are placed in safe and stable placements in residential care. The Case Manager oversees the provision of services delivered to each of the 8 contracted clients in residential care and works directly with the care team of professionals involved in each case. The Case Manager works closely with the young people’s families and aims to strengthen relationships.

The Position: 

The full time Case Manager role will work alongside a multi-disciplinary team in the delivery of our residential care service. The position objectives are:

  • To guide residential youth workers to provide young people with safe and stable residential care placements that will support their development.
  • To ensure the provision of high quality services to young people and their families who are involved in the residential care program.
  • To coordinate individual care teams to ensure young people access services they require.  

To Be Successful in this Position You Will Need:

  • Tertiary qualifications, preferably in Social Work / Psychology or equivalent.
  • To have experience in Case Management.
  • To work effectively within a multi-disciplinary team of professionals in the delivery of our residential services program.
  • To be flexible and adaptable and be able to work outside normal office hours on occasions.

Please Note: It is mandatory for the successful applicant to hold a current Working with Children Check and be prepared to undergo a National Criminal History Check prior to commencement.
