The Care Hub is a new initiative that brings together a number of organisations to form an integrated team of professionals to support Children and Young People in the areas of Mental Health, AOD, Cultural Connection, Family Work, Community Engagement and Therapeutic Support.
The team will be based from a single location in Bendigo, forming a collaborative, integrated team of staff helping support those entering care for the first time. The aim of the Care Hub is to provide a wraparound service and team of staff to support effective assessment, planning and intervention to help support the Children, Young People and Families through the process and ensure all needs are met.
The integrated team will include professionals from AV and a host of other partner organisations, all working together to do better.
In other programs such as The Orange Door, Integrated and Collaborative teams have proven to be hugely effective in providing wrap around support and services to clients, as well as great support structures and ongoing learning for staff.
The Key Practitioner acts as the key worker for new DFFH referrals of Children and Young people into the Care Hub. Our practitioners will use the Best Interest Framework (Along with other approaches) to comprehensively assess risk, needs and required supports, supporting vulnerable families and all key stakeholders to ensure the safety, stability and wellbeing of Children and Young People.
This will require strong case coordination and collaboration with Care Hub colleagues, Children, Young People and Families and the DFFH as well as strong engagement and support delivered through in home and community outreach activity.
