Your role:

As the Lead Practitioner it is your role to liaise with DFFH, monitor risk and safety plans and identify performance trends and areas for improvement.

Future Pathways is part of AV’s approach to the Department of Families, Fairness & Housing’s (DFFH) state-wide initiative to support Family Preservation & Reunification (FPR). The Future Pathways team compliment a suite of family supports provided within the FPR model. 

The Future Pathways program is an innovative approach built on practice elements such as motivational interviewing, communication skills building and goal setting to respond to identified needs and achieve real and demonstrable outcomes.

About you:

  • A practitioner who has previously lead teams or provided secondary case consults
  • Ability to reflect on practice and spot gaps in service delivery
  • Provide coaching and feedback
  • Previous experience in a therapeutic environment ideally utilising recognised therapy practices such as motivational interviewing
  • Knowledge and experience working with families with complex needs, mental health, AOD, family violence
  • Willingness to work outside of normal hours to see families together and at home
  • Ability to engage quickly and assess risk
  • Experience in out of home care would be beneficial

Qualifications:

  • A relevant tertiary qualification in Social Work, Psychology, Early Childhood Specialist and/or related behavioural sciences at degree level with relevant experience; or diploma level with substantial experience
