As the Lead Practitioner it is your role to liaise with DFFH, monitor risk and safety plans and identify performance trends and areas for improvement.
Future Pathways is part of AV’s approach to the Department of Families, Fairness & Housing’s (DFFH) state-wide initiative to support Family Preservation & Reunification (FPR). The Future Pathways team compliment a suite of family supports provided within the FPR model.
The Future Pathways program is an innovative approach built on practice elements such as motivational interviewing, communication skills building and goal setting to respond to identified needs and achieve real and demonstrable outcomes.
