During this time we have established a solid reputation for our high quality, personal and flexible services. Our Support team members are directly employed by annecto and are our most valuable resource.

The Position

We are looking for another Case Manager due to our significant growth in the region!

This is an exciting opportunity to work with an independent, not-for-profit organisation located in Mawson to work with a small, friendly and client-focused team. As well as Case Management with our Home Care Package clients you will also be actively involved in service promotion, providing advice on access to community-based aged care to community members and working closely with our team of wonderful direct Support team members. We also provide Short Term Restorative Care, CHSP Other Food Services, brokerage to other agencies and private fee for service. Variety is assured in this rewarding position!

The ideal candidate should have:

  • Demonstrated previous experience in client assessment and Case Management in a community setting, preferably in aged care;
  • Demonstrated knowledge of, and ability to access a range of community resources and professional services for clients;
  • Demonstrated ability to work effectively with a culturally diverse group;
  • Demonstrated ability to work through challenges in systematic and creative ways;
  • Demonstrated ability to work autonomously with a high degree of initiative and attention to detail;
  • Demonstrated ability and confidence to actively promote services to build client base is highly desirable;
  • Excellent communication skills, both written and verbal;
  • Proficient in the use of computer software: Word, Excel, Internet and email;
  • Demonstrated commitment to the ‘Purpose and Principles’ of annecto – the people network;
  • A current valid Australian driver’s licence

What would be great to have?

  • Experience using Client Management Systems such as Techone, Alayacare
  • An understanding of the ageing process and its cultural impact;
  • An ability to speak a community language/s.
