About the role?

The Case Manager will report to the South East Coordinator, Home care and work within the Home care packages team. They will assist and support eligible recipients who have complex care needs and provide solutions and alternatives to residential care.

The incumbent will also promote engagement with all key stakeholders, and ensure homemade packages are planned, implemented and monitored in consultation with individuals, their carers, service providers and other significant people in the person’s life.

Requirements:

  • Tertiary qualifications in relevant health/ social sciences
  • Demonstrated commitment to the Purpose and Principles of annecto - the people network;
  • Previous experience in case management in a community setting
  • Demonstrated ability to work efficiently with culturally diverse groups
  • Demonstrated ability to work autonomously with a high degree of initiative;
  • Excellent communication skills, both written and verbal.
  • Understanding and commitment to implementing Consumer Directed Care Initiatives and person centred approaches;
  • Current unrestricted Victorian Drivers licence.

What would be great to have?

  • Knowledge of local/regional community resources/professional services to support older and vulnerable Australians;
  • Experience working in a similar capacity in a community based not-for-profit organisation.
