Our justice-focused program(s) provide a platform to enhance existing NDIS services and develop novel support coordination and case management solutions, streamlining treatment pathways and improving client outcomes. You will play a critical role in this through strategic stakeholder engagement, organisational development planning, co-design and co-production of our services. The hybrid NDIS Support Coordinator/TAC Case Manager is responsible for the day to day service provision ensuring the highest quality support for the people who access the program(s), providing:
You will be the key organisational contact for all National Disability Insurance Scheme (NDIS) and Transport Accident Commission (TAC) referrals, service provision and performance reporting across NSW. A fundamental component of the role will be to explore and identify opportunities for continuous quality improvement initiatives, strategic partnership development and organisational growth through project implementation and management methodologies.
