We have an exciting opportunity to join the South East Sydney Regional Assessment Service with Aspire4Life as a Home Support Assessor. In this role, you will conduct assessments using a holistic and client centred approach, with a focus on wellness and reablement. These assessments will support older Australians to continue living independently within their chosen community, by considering needs such as assistive technology, social support services, and community care. You will have the opportunity to travel throughout the South East Sydney Aged Care Planning region to work collaboratively with clients in their homes.

This position will commence 1st November on a fixed term contract until 30 June 2022 (with the possibility for renewal based on funding). Candidates will need to be centrally located within the South East Sydney aged care planning region. This is a full time opportunity working Monday to Friday (no weekend or shift work!)

We offer a fantastic 2 weeks of full-time training and orientation at our head office in Coolangatta, QLD (all expenses covered), commencing September 2021, however due to the current border restrictions, orientation may be held virtually.

A typical week as an assessor with Aspire4Life:

  • Travel throughout the South East Sydney region, visiting clients in their homes as you utilise your assessment experience and expertise to conduct holistic style assessments, with a focus on wellness and reablement
  • Capture vital client information to build on the My Aged Care client story and develop an individualised support plan underpinned by the clients unique goals
  • Develop strategies to connect people with appropriate services and supports in their community (may it include assistive technology, social support or community care) to further empower the client with their independence

You will be supported by our vibrant centralised liaison team, who coordinate with the clients and schedule in the appointments – you will not need to worry about planning travel distances or timings! Whilst you are out on the road, your regional lead will be there to provide support and guidance.

Need to be/have:

  • Experience in assessment and goal oriented support planning in aged, community, health or social services, and previous experience working in aged care, community services, assessment and/or coordination of services
  • Confident communication and interpersonal skills, with the ability to engage and motivate clients, families, staff and other stakeholders with a positive approach
  • Some local knowledge and/or the ability to network with community aged care service providers and stakeholder within your region

What to bring?

  • Tertiary qualification as a Registered Nurse, Social Worker, Occupational Therapist or Physiotherapist (highly regarded) or minimum 3 years experience working in the aged or community care sector in a similar role, with relevant qualifications
  • Current AHPRA Registration, or relevant (highly regarded)
  • Unrestricted Australian driver's licence and own reliable insured vehicle (and be comfortable travelling long distances by car)
