The Street to Home program supports people who are homeless to live safely and develop pathways out of homelessness.
In this role, you will provide mobile, outreach and centre based case management support to individuals and families experiencing vulnerabilities in Townsville.
You will operate from a strengths based and capacity building framework to ensure the promotion of social inclusion, human rights, participation and consumer choice.
