This complex and challenging rare genetic disease affects about 2,500 people in Australia and frequently causes severe epilepsy, autism and intellectual disabilities.

What you'll be doing

The TSA telehealth nurse which is currently part of the Patient Pathways research program (cc-dr.org/au/pathways), provides critical one-on-one support to members of our TSC Community.

The role includes:

• Providing telephone support to assist patients, their families and carers to help them navigate through the healthcare system and access all the existing services that are available to them, including developing or reviewing care management plans
• Providing specialist information and advice regarding TSC to patients and families (TSC-related training will be provided)
• Liaising with other relevant health care professionals
• Keeping abreast of developments with respect to TSC-related research and trials and providing the TSC community with regular updates on clinical trials and lay summaries of relevant and emerging research
• Attending (occasional) relevant TSC-related forums
• Contributing to the development and delivery of TSC-related education programs and resources for patients, their families and other health care professionals
• Participating in data collection and health outcomes reporting for the Patient Pathways pilot

Selection criteria

• Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA)
• Bachelor of Nursing degree or evidence of further study
• Minimum of 3 years’ full time equivalent post registration experience within Australia, ideally in areas relevant to TSC, such as epilepsy, autism, renal, cardiac and intellectual disability
• Experience in telehealth and care coordination highly regarded

• Able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations
• Proficient computer and technical skills, including use of databases (we use Salesforce and CiviCRM) and Microsoft Office (specifically Outlook, Powerpoint, Word and Excel)
• Ability to work autonomously but also contribute to the overall TSA team
• Highly organised and well-developed interpersonal, oral and written communication skills
• Confident to communicate and collaborate with other community health care organisations and personnel
