We are seeking a permanent, part time (4 days per week) Case Manager for the Housing Services team that sits within the Education Early Intervention Services and Housing Support program. The Case Manager - Housing Services will provide case management to clients, with or without children, who are experiencing homelessness or at the risk of becoming homeless.
Clients who access this service are often in crisis and have multiple, complex needs making them vulnerable and as such the program aims to identify and address the issues that have led to their current situation. The program focuses on principles of social inclusion and social justice to ensure that those who have been overlooked by every other element of the system have a place to go.
The Case Manager will support clients to access appropriate accommodation and develop independent living and coping skills for a sustainable future, wellbeing and community connections. The objective of the position is to increase the client's capacity through case planning as well as identifying and creating opportunities for early intervention and prevention that support young people, adults and their children who have experienced homelessness.
