Family Services Specialist Disability Practitioner roles have been established to build both disability and National Disability Insurance Scheme (NDIS) expertise across the community-based family services system. The roles will assist vulnerable children and parents with disability to access disability supports and participate effectively in the NDIS.
The role will also be responsible for providing disability-related consultations to the broader family services system within Latrobe and Baw Baw Area and work alongside the other Specialist Disability Practitioner in South Gippsland and Bass Coast to make up the Inner Gippsland Area.
This position is based in Morwell with a hybrid of in office and working from home.
The Specialist Disability Practitioner role involves the provision of effective case management and support for children, young people and their families. This is achieved through the provision of case work, parenting strategies, tailored Family Action Plan and referral/networking to other service providers including child protection.
The role provides specialised in-home case management and engagement of vulnerable families, providing parenting skills, professional development, secondary consultation, and referrals.
You will hold a minimum Diploma level qualification in Social Work, Psychology, Disability, Welfare or a related discipline along with experience in working with families in the health and welfare sector as well as NDIS.
You will have specialist knowledge and experience in working with vulnerable children and families from a diverse background and presenting with a variety of issues and needs, including resistance to engagement with professionals and an understanding of the issues confronting children and families involved in the child protection system.
