How you'll transform lives

The Justice Housing Support Program (JHSP) is funded by the Department of Justice to provide housing and support to specifically referred clients that are entrenched in the criminal justice system and have a history of homelessness. The objective of JHSP is to reduce the ongoing homelessness and re-offending among court support participants by providing linked transitional housing and support to accused persons on the court support programs who are homeless or have a demonstrated history of homelessness. This client group presents with coexisting multiple and complex needs such as mental health, problematic substance use and family/relationship breakdown.

As a Case Manager, you will provide initial housing assistance, including assistance with tenancy sign up; assisting with long term housing exit plans; and be the point of contact with Transitional Housing Management (THM) providers and Office of Housing in regard to exit planning from transitional accommodation.

Reporting to the Justice Housing Support Coordinator, you are expected to:

  • Direct Service delivery in response to referrals from Magistrates courts, you will engage with clients to help and to create housing case plans.
  • Provide assertive outreach and case management to clients residing in allocated THM properties.
  • Work with clients in regards to basic needs such as housing, income entitlements, employment, food, clothing, domestic services and medical care. This may include referrals to specific services and/or direct provision of services. Services are provided at the direction of clients.
  • Advocate on behalf of clients with other agencies to ensure access and delivery of services. This may require the worker to accompany the person to appointments.
  • Manage own case load and day-to-day schedule of client appointments, meetings and other tasks.

  • Experience in the social welfare sector and working with clients with complex mental health issues
