Launch Housing is a secular Melbourne-based community organisation that provides homelessness services and life-changing housing supports to disadvantages Victorians. With over 400 staff working across 14 locations, we want to end homelessness and are committed to creating lasting societal change to help those most in need in our community. What you'll do:

The Northern Outreach Team provides a range of flexible, responsive and professional services for people experiencing homelessness or who are at risk of being homeless in Melbourne's northern suburbs. We actively engage clients to assist them to gain and maintain sustainable housing and other supports. The program works in partnership with other services to keep people safe, build rapport, and engage them with the broader service system and their community.

As a Case Manager, you will report to the Northern Outreach Coordinator and will:

  • Provide case management and support to clients with an aim to stabilise and sustain housing and prevent homelessness
  • Engage with clients to improve their quality of life and social inclusion in the community
  • Ensure clients maintain safe, appropriate and sustainable accommodation
  • Work in an assertive outreach capacity with clients assisting them to provide practical support and make appropriate referrals

Who you are:

You are kind. Passionate. Empathetic. You're meticulously organised and your high-level communication skills see you negotiating difficult and confronting situations with poise, clarity, and respect. You have experience working in the mental health, homelessness or alcohol drug dependency sector, you understand the implications of the complex mental health issues on loved ones, friends, and family as well as the individual. You're here to help.

You're also organised, meticulous with detail and value friendship, teamwork and hard work.
