About the role

A unique opportunity to support the Commonwealth Home Support Programme Consortium (Healthy@Home) and lead collaborative and strategic activities across the Aged and Community Care team. Current funding for this role is until 30 June 2022.

Duties and Responsibilities

  • Lead projects within the Healthy Ageing team from planning and development through to realisation
  • Support consortium aged care service providers to implement quality improvements
  • Establish and maintain collaborative working relationships with relevant internal and external stakeholders; including engaging with consumers, carers, community and healthy@home consortium members
  • Strengthen capacity of the community aged care sector to overcome challenges, respond to reform and build partnerships in improving continuity of care for people in the region
  • Stay abreast of national aged care and related health policy changes; convey relevant information within the Aged and Community Care team and healthy@home consortium; and identify implications and proposed activities or responses
  • Identify business development and innovative opportunities and write project and funding proposals in the context of national aged care reforms and to address identified needs.

Skills and Experience

  • Tertiary qualifications in a health or social care related field (highly desirable)
  • Experience and knowledge of primary health care, acute health and the aged care sectors (highly desirable)
  • Experience in leading/managing projects, creating change and improving outcomes for consumers
  • Experience in analysing and synthesising complex information and presenting verbally and in writing via briefing notes, reports, presentations
  • Experience in building and maintaining collaborative relationships with a broad range of stakeholders
  • High level interpersonal and written communication skills
  • Advanced skills in Microsoft Word, Outlook, Excel and PowerPoint.
