In this exciting newly created position you will be focussing on wellbeing and the development of children with a client-centred, goal-focused approach with a direct focus on those with a disability. Family Case Managers, Practitioners and Group Facilitators work with vulnerable children, young people and their families who may be impacted by trauma, substance abuse, mental health or family violence and are potentially at risk of becoming involved with the Child Protection system or are already involved with child protection.
Family Case Managers work across a range of families and locations providing longer term support, consultation and group facilitation with a specialist disability focus. The focus of all work with families is to ensure the safety and wellbeing of children and to keep them safe and secure.
As the Advanced Practitioner you will work closely with the Team Leader and team that are solely focussed and experienced in Family Case Management and Disability by applying your specialised knowledge, guidance, advice and recommendations through your experience and practice. You will assist the Team Leader to support the team in the delivery of a high quality, responsive and effective service within a strong and professional risk assessment framework. You will be driven to assist families to achieve long term positive outcomes and also those where a disability is present.
This unique position has been created to increase our support for vulnerable families, with a particular focus of supporting those family members who have a disability. This is a rare opportunity to join experienced and like minded individuals who have a passion for supporting families with children with an extra focus on disability.
Tertiary qualifications in Social Work, Welfare Studies or their equivalent, or other tertiary qualification appropriate to public welfare practice.
You are an enthusiastic and committed individual with demonstrated skills and experience working with vulnerable families and working collaboratively to achieve positives outcomes. Your highly developed cultural awareness and sensitivity, partnered with excellent communication skills, will enable you to quickly build strong relationships with our clients and key stakeholders. You are not afraid of a challenge and constantly seek to grow and develop.
