About the Role – no work travel required!

We are seeking an experienced, enthusiastic community Case Manager/Client Adviser to provide independent, ongoing advice and guidance to our clients across Victoria and New South Wales. This is a full time, max-term, Parental Leave role - till 4/7/2022 - possibility of extension. It is an office based job 2 days per week and 3 days WFH – no on the road travel required!

As a Case Manager/Client Adviser, you work collaboratively with clients of both Commonwealth Home Support Program (CHSP) and HACC PYP Linkages, their carers and representatives, developing flexible, personalised and culturally appropriate care plans.

Duties include:

  • Review intake information engage new clients, establish the client agreement, care plan and connect the client with services and supports locally.
  • Conduct annual reviews for all clients, revising their care plan, goals, services and, for CHSP clients, identifying areas for reassessment through My Aged Care
  • Practice person centred care planning, provide independent advice and guidance to clients and their families
  • Support the day to day functions of CHSP and HACC PYP Linkages services.
  • Network with Care Connect employees, stakeholders, partnered service providers on behalf of the client and Care Connect

To be successful in the role you have:

  • Excellent interpersonal communication, networking and negotiation skills
  • An ability to take the initiative, prioritise, delegate and organise activities
  • Experience with person-centred care planning processes, including assessment and establishing client goals
  • Experience managing CHSP and/or HACC PYP Linkages funded services
  • Ability to work with set KPIs and achieve targets
  • Experience in the Microsoft Office suite and exposure and confidence to acquire skills for in-house client database systems
  • Availability to work across 2 programs (for a short period) - CHSP & HACC PYP Linkages

Highly regarded, but not essential

  • Knowledge of the Commonwealth Home Support Program (CHSP), HACC PYP Linkages and Aged Care standards, community resources and service networks for older Australians and their carers

Qualifications required for the role:

  • Tertiary qualifications in allied health, social science, health/welfare discipline, or a related field.

Additional requirements:

  • All appointments are made subject to a satisfactory National Police Check conducted by Care Connect and a valid Working with Children Check provided by the applicant, as requested
  • A statutory declaration is required, confirming residency in a country other than Australia Depending on the information provided an international police check/s may also be required
  • Part of our selection process requires the completion of psychometric assessments
