As the Carer Gateway Service Provider (CGSP) in Region 4 of NSW (Central Coast, Hunter New England and North Coast) we have 2 full time opportunities for Aged Care Intake & Assessment Officers in our office in North Sydney. This is a contract role until June 2023, with possible extension.

Aged Care Intake & Assessment Officer:

These positions are responsible for managing referral calls from My Aged Care to the Carer Gateway. The Aged Care, Intake & Assessment Officer will support the carer through the Carer Support Planning Process including an intake, registration and needs assessment, support planning, coordination, support and monitoring to ensure carers can be holistically supported by understanding their needs and appropriately referring them for supports.

Reporting to the Team Leader, Aged Care Specialisation Carer Gateway you will be able to demonstrate the following essential criteria:

  • Qualification in Social Work, Counselling, Community Services, Social Science or other relevant discipline and/or currently studying and/or able to show demonstrated work experience
  • Understanding of the Community Services sector, specifically Carers and an understand of the My Aged Care framework
  • Experience and knowledge in direct service delivery
  • Advanced communication skills with experience speaking to people from diverse backgrounds
  • Demonstrated experience in telephone support work
