Coast & Country Primary Care (CCPC) is an independent not for profit organisation, providing programs and services that support the provision of primary healthcare and community services to the Central Coast community. Based in Erina, we offer a range of programs and services across the Central Coast.

The Way Back Support Service provides practical, non-clinical support following a suicide attempt. The service aims to assist individuals and their families by linking them with appropriate support services to help manage life situations and challenges. CCPC is looking for resilient people who are emotionally intelligent to provide professional support to people in their most vulnerable stage of life. About the role

We are looking for a mix of part time and full time WBSS Support Coordinators that will be responsible for the coordination of services in accordance with the guiding principles of the WBSS by providing high-quality aftercare to support of high-risk suicidal clients to stay safe. This role aims to help build capacity of individuals to self-manage distress and improve mental wellbeing; improve links with clinical and community-based services to meet individual needs and circumstances and increase social connectedness and links to supportive networks such as families, friends, peers, and carers.

Responsibilities Include:

  • Actioning all referrals in line with the WBSS service delivery model
  • Confirming eligibility of all referrals
  • Implementing service delivery tools for each client
  • Providing assertive outreach support for all consenting clients
  • Making and/or advocating for referrals to community-based services on behalf of all clients
  • Complying with all data entry and client information requirements
  • Effectively communicate with GPs and other referrers regarding the status of referrals, as well as reviewing decision making as it relates to referral pathways for clients
  • Assist ineligible clients to access more appropriate services as needed in conjunction with their referring GP or provisional referrer
  • Identify and escalate referrals where significant risk factors and/or complex issues are required to be reviewed and/or assessed by the Team Leader

Essential Selection Criteria:

  • Certificate IV qualification in Mental Health or equivalent; and / or extensive experience
  • Experience and commitment to supporting people in crisis with mental illness and complex needs
  • Demonstrated communication skills with clients, families and/or carers with a high level of discretion and sensitivity when dealing with confidential matters
  • Demonstrated relationships, engagement and networking within the Government and non-Government, public and private health, and community service sectors
  • Proven ability to meet key performance requirements with excellent time management skills
  • A valid Working with Children Check for paid employment to be obtained and maintained at your own expense
  • Available to work flexible hours
  • Eligibility to work in Australia on a permanent basis
  • Driver’s license and access to a reliable and insured motor vehicle

  • Lived experience of mental health and recovery
