We are seeking a Group support worker to facilitate activities and social engagement within a Social Support program.

About You

The Social Support Service provides a variety of beneficial programs that are on site or excursion based in the community. The target groups for the program are frail aged adults, clients living with dementia and other health conditions and adults with a disability.

Reporting to the Social Support Coordinator, the Group Support Worker will work in a small team to deliver group social support services in accordance with the policies and procedures of the Connect Health & Community. You will be creative, caring and able to demonstrate a passion for working with older people.

We are looking for:

  • Minimum Certificate III in Lifestyle and Leisure or similar
  • Experience in Planned Activity Groups/Social Support for older people in the community or with Lifestyle Programs within residential aged care
  • Food handlers certificate (or ability to complete as soon as possible)
  • First Aid certificate (including CPR)

The period of employment will commence as soon as available. It is a part-time role of 21 hours per week over three days, ending on 30/6/24, however with the potential to extend.

Salary and conditions as per the Community Health Centre (Stand Alone Services) Social and Community Service Employees Multi-Enterprise Agreement 2017-2020.

A condition of employment is a current Working with Children’s Check and Police Check. If you do not already have these checks, you must be prepared to apply for and fund them.

Under the COVID-19 Mandatory Vaccination (Specified Facilities) Directions, Connect Health are required to collect, record and hold vaccination information about all Workers and are not able to permit a worker who is unvaccinated to work onsite unless an approved Medical Exemption applies.
