What will the role look like;

As part of the Hoarding and Squalor Team, you will use your extensive skills and experience to oversee the Pathways of Recovery & Care of Aged Care clients in the Hunter, Inner West, North Sydney, Western Sydney and South East Sydney impacted due to Hoarding and/or Squalor. Ensuring that they have a flexible, responsive service tailored to their individual needs and interests. With a focus on Allied Health specialised support to prioritise clinical and non-clinical needs.

  • Engage with clients to undertake a comprehensive assessment, whilst developing a trusting and professional relationship that supports clients to reach their mental health goals
  • You will be responsible for ensuring the delivery of flexible and responsive services tailored to an individual's needs and interests in accordance to program guidelines, standards and policies, whilst also providing advocacy and referral services to clients.
  • Develop and implement support in response to the impact of the Hoarding and/or Squalor on the individual’s mental health and wellbeing
  • Collaborate with other supports in the planning of comprehensive support, risk management, crisis management and exit plans with the clients individual Recovery Plan/Support Plan
  • Encourage and support clients to access and remain engaged with their psychosocial rehabilitation, physical health care and social support services.
  • Develop effective relationships within the team and maintain partnership with external stakeholders to ensure clients receive the highest quality, coordinated service.
  • Work to support collaboration with clinical mental health services and other key stakeholders to achieve program outcomes
  • Actively participate in team meetings, decision-making processes and service planning sessions, supervision and staff development activities.

Who you are?

  • The ideal candidate is a diligent, empathetic, and driven individual who is motivated by their passion to assist others who are vulnerable members of the community
  • Relevant tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Nursing, Psychology or Counselling.
  • Demonstrated experience in Mental Health case managing is essential to be considered for this position.
  • Understanding of the complexities associated with situations of Hoarding and/or Squalor highly desirable
  • Highly developed planning, managing and program implementation skills
  • Understanding of Aged Care services and referral pathways
  • High degree of computer competency with an ability to learn new software programs including Orca & Procura
  • Registered and insured motor vehicle and a current unrestricted Australian drivers' licence
  • Covid vaccination record
