About the role

This exciting opportunity sits in the Connected Communities Strategic Development team as part of the Community Life division which strives to develop healthy, active, connected and safe communities through the delivery of community facilities, events, experiences and life cycle services from birth to older age. We are looking for someone who is innovative, compassionate and caring and committed to ensuring clients have access to the supports they need.

Responsibilities of the case manager role include:

  • Providing case management and advocacy support for older people who are not supported by the Home Care Package program and are experiencing significant difficulty with accessing the supports and services they need to live a healthy and connected life.
  • Facilitating and building relationships with other community-based agencies to improve the quality of access to services for all stakeholders and influence systemic changes necessary.
  • Providing outreach support, assisting and facilitating client’s connections to services as required.
  • Being the primary point of contact for consumers, carers, service providers and other health professionals

About you

  • Have tertiary qualification in Allied Health/Health or relevant other qualifications with substantial experience working as a case manager/worker in an outreach capacity.
  • You will be experienced with understanding the range of health issues and support needs that older people can experience.
  • Have experience using a strength-based approach to support older people/using trauma informed therapy highly desirable.
  • Will be able to demonstrate a clear understanding of case management processes including using a flexible approach to complete assessments and the importance of providing advocacy support
  • Posses a positive growth mindset that works collaboratively with other services to improve client outcomes.
  • Have outstanding communication skills that can engage with a diverse range of clients and their carers
  • Willingness to undergo pre-employment background checks including: National Criminal History Check, Working with Children Check and Eligibility to Work check.
  • Covid-19 Vaccination in line with State Government directives.
