• Full Time, Temporary to June 2022
• Salary Range: $68,463 - $79,776 + super

In this newly created role, you will support individuals to connect with community groups and resources, social support activities and services. You will develop personalised care and support plan to improve health and wellbeing by reconnecting people with appropriate community groups.

You will have experience in providing support to individuals or families to improve their health outcomes through linking with social services and supports. You will also have comprehensive knowledge of the range of services and supports available in City of Greater Bendigo region and develop strong links with local agencies.

If you hold tertiary qualifications in social work, allied health, nursing or Graduate Certificate in Community Services, or similar discipline, we welcome your application. A satisfactory police records check and Working with Children Check is required for this position.
