The Lifestyle Support Worker provides high-quality, person-centred support for consumers, while working as part of the EADC team. The Lifestyle Support Worker will work with consumers with low to complex needs and their carers toward achieving individual care plan goals while meeting the group's needs as a whole.
The role requires skills in working collaboratively with older people, including people with dementia and people with disabilities, and their carers. Encouraging client independence and participation in their community is also required.
The Lifestyle Support Worker works in both our Social Support program and in our Flexible Respite program.
