About the role

The Lifestyle Support Worker provides high-quality, person-centred support for consumers, while working as part of the EADC team. The Lifestyle Support Worker will work with consumers with low to complex needs and their carers toward achieving individual care plan goals while meeting the group's needs as a whole.

The role requires skills in working collaboratively with older people, including people with dementia and people with disabilities, and their carers. Encouraging client independence and participation in their community is also required.

The Lifestyle Support Worker works in both our Social Support program and in our Flexible Respite program.

Your daily duties will vary but will include:

  • Design and deliver meaningful group activities
  • Provide Flexible Respite services to carers in their preferred location
  • Document daily client file notes and record attendance in the client management system (TrakCare)

Who are we looking for?

  • Hold a minimum qualification of Certificate III in Aged Care or Equivalent
  • Certificate IV in Leisure and Health/Lifestyle & Leisure
  • Experience in planning and delivering Social Support programs for older people with varying needs.
  • Experience or be confident to independently deliver Flexible Respite services in the community that consider the needs of the client and carer.
  • Well-developed interpersonal and written communication skills
  • Confidence in using technology
  • All Healthcare workers in Victoria are required to be vaccinated against COVID-19, you will need to provide evidence before commencing employment with Cohealth.
