About The Way Back Service

Communify, with funding from Brisbane North Primary Health Network will be delivering in partnership with Brisbane Youth Service, Institute of Urban Indigenous Health and The Way Back Support Service (TWBSS) in the Brisbane north region. TWBSS is a suicide attempt after-care service that was developed by Beyond Blue to fill the gap in continuity of care for people discharged from hospital after a suicide attempt. TWBSS provides non-clinical, tailored support for up to three months following discharge from hospital after a suicide attempt or suicidal crisis.

About the role

  • Provide leadership, coordination and guidance
  • Support the planning, development and implementation of suicide prevention activities with participants
  • Engage participants and develop trust, culturally appropriate, trauma informed professional relationships
  • Work collaboratively with participants to identify their needs, set goals and develop safety plans and care plans
  • Work in partnership with service providers and community groups to create a support network for participants

Role criteria

  • Tertiary qualifications in Mental Health or Social Services related fields
  • Demonstrated management experience in human services sector
  • Sound understanding of personal, family and social issues associated with suicidal ideation and attempts, including awareness of groups identified to be at risk of suicide
  • Demonstrated skills and ability to work effectively with participants, family members and service providers to identify solutions
  • Excellent communication skills, particularly with individuals and their families/carers who may be experiencing significant distress
  • Experience liaising with external service providers including tertiary mental health services, GPs and other community-based services and organisations
  • Solid understanding of recovery principles, strengths based principles and trauma informed support
  • High level of competency using technology

  • National criminal Record check - disclosable outcomes considered
  • Open Queensland Drivers Licence
  • Double dose COVID 19 vaccination record
