Temporary Contract – Flexible Working Hours
The Intensive Family Support Program is a pilot project aimed at developing a methodical and measurable approach to intensive support coordination for families designed to provide practical and emotional support for families impacted by inter-generational trauma.
This project takes a single case plan approach to working with families experiencing multiple and/or complex needs and actively collaborate with other agencies to ensure families get the services and support they need to achieve their case plan goals.
Services engage with all family members who are willing to receive support. This role will support families to link with services that will help them improve their wellbeing and assist them to overcome barriers preventing them from living healthy and productive lives.
The position, based in Kalumburu, will require regular contact with families using intensive support coordination. It will also require extensive networking and collaboration with service agencies to link families with targeted services and facilitate connection with clinical and intervention therapies.
