This important and exciting opportunity builds on previous palliative and end of life models of care work to develop and implement new and innovative palliative care projects or expand on existing activities, in order to improve consumer experience and access. The Project Coordinator, Palliative Care is responsible for this vital work in SNSW.

About the Role

Drive awareness of palliative care and facilitate improved access to, and coordination of, safe quality integrated palliative and end of life care services and ensure patients/carers palliative care choices are met. These roles will work with local stakeholders to develop and implement person centered initiatives that:

  • Improve access to palliative care at home and support end of life care systems and services in primary health care and community care,
  • Enable the right care, at the right time, in the right place to reduce unnecessary hospitalisation,
  • Generate and use data to support continuous improvement of services across sectors,
  • Use available technologies to support flexible and responsive palliative care at home, including after hours.

About you

  • Relevant tertiary qualifications and experience in aged care, end of life care, or primary/community health settings,
  • Demonstrated ability to engage with and work collaboratively with a wide range of internal and external stakeholders to achieve innovative and practical solutions,
  • Ability to work unsupervised, under pressure, prioritise commitments and meet critical deadlines while maintaining quality standards,
  • Demonstrated skills and experience in delivering projects in complex and specialised environments,
  • An affinity with the purpose and the values of the organisation.
