COORDINARE is the values led Primary Health Network for South Eastern NSW and is one of 31 PHNs across Australia and one of 10 PHNs in NSW. Our region stretches from Helensburgh in the north, past Eden to the Victorian border in the south, and inland to Cooma/Monaro, Queanbeyan, Yass and Goulburn.

This important and exciting opportunity builds on previous palliative and end of life models of care work to develop and implement new and innovative palliative care projects or expand on existing activities, in order to improve consumer experience and access. The Project Coordinator, Palliative Care is responsible for this vital work in SNSW.
Drive awareness of palliative care and facilitate improved access to, and coordination of, safe quality integrated palliative and end of life care services and ensure patients/carers palliative care choices are met.
