About the Role

Following a successful two-year pilot, Darebin City Council is seeking two highly experienced and engaging professionals to deliver its Aged Care Navigation Service, Positive Ageing. Situated in the Aged and Disability department, the Positive Ageing Officers are responsible for delivering high quality, age friendly navigation support to mitigate community impacts of system change and enable older residents to age well in Darebin.

  • Provide accurate advice and support the informed choice of older people and carers in relation to aged care system navigation
  • Provide short term care coordination for older people with specific challenges or vulnerability indicators
  • Engage with internal and external stakeholders to build partnerships to extend the service’s reach
  • Review, develop and deliver the Support for Carers Program
  • Contribute to evaluation activities including documenting and analysing data

Skills & Experience

  • A tertiary qualification in allied health or related discipline and/or relevant experience in Case Management, Aged Care Assessment
  • Sound knowledge of the aged care service system, social determinants of health in older age, policy directions within the aged care sector and Age-Friendly principles
  • Knowledge of, and experience in strengths-based approaches providing information, decision-support, linking advocacy and casework for older persons with vulnerabilities
  • A current valid Victorian Driver's Licence
