As an NDIS Support Coordinator you will assist participants to understand and implement their NDIS plan by linking the participant with services that meet their individual needs and monitor their NDIS plan outcomes and expenditure. Support Coordinators are the face of Darebin Support Services. As such in this role you will be developing professional relationships with NDIS participants, families and other service providers.

Role Overview

The primary responsibility of the Support Coordinator / Case Manager is to provide tailored coordination services for participants funded by the National Disability Insurance Scheme (NDIS), Transport Accident Commission (TAC), and other funding bodies. Your key focus will be to empower participants by enhancing their ability to manage and coordinate their supports, while building their capacity to make informed choices and take control of the services they need to lead fulfilling lives and engage actively in their community.

  • Empowerment through Coordination: Assist participants in developing the skills and confidence needed to effectively manage and coordinate their supports.
  • Capacity Building: Support participants in building their capacity to make informed decisions and exercise control over their lives, ensuring they have the resources to live independently and meaningfully engage with their community.
  • Person-Centred Practice: Adhere to a person-centred approach that prioritises the participant’s autonomy, ensuring that all supports are aligned with the goals and outcomes specified in their approved plan, within a safe and supportive environment.
  • Implementation of Supports: Facilitate the implementation of the participant's plan by coordinating a range of supports, including funded services, mainstream and community resources, and informal supports, ensuring these are driven by the participant's individual needs and aspirations.
  • Operate within the NDIS framework: to ensure that all services are aligned with the participant’s approved plan and that coordination efforts are centred around the participant’s goals.
  • Collaborate: with participants, their families, and support networks to ensure that all services and supports are integrated, cohesive, and reflective of the participant's desires and needs.

Skills, Experience and Qualifications

Required:

  • Current NDIS Worker Screening Clearance
  • NDIS Worker Orientation Module Certificate
  • Working with Children Check
  • Diploma Community Services or equivalent experience
  • Current First Aid including current CPR
  • Current driver's license
  • Victorian Police Check
  • Infection Control Certificate
  • Knowledge of Microsoft Office Suite, with a proficiency in Word and Outlook
  • Experience in the disability sector
  • Demonstrated high level of verbal communication and interpersonal skills
  • Positive, empathetic and willing to learn new skills
  • Adaptable to an ever-changing disability landscape
  • Time management and autonomous working skills

Preferred:

  • Experience as a Disability Case Manager / NDIS Support Coordinator
  • Understanding of the NDIS and its operational guidelines and framework
  • Detailed report writing skills
