A little about the role 

The Community Asthma Educator will work as part of the Community Asthma Program Team (CAP) based at DPV Health within a client centred framework providing individualised Paediatric Asthma Education and Support to children, young people and their families.

As part of the CAP Team you will operate within a multidisciplinary team to deliver quality, innovative client directed outcomes, supporting carers and stakeholders, as required to ensure children and young people have decreased asthma related hospital presentations and increased quality of life.

Your day to day responsibilities will include:

  • Providing evidence based, person centred, goal-directed assessment, education, care planning and self-management support services to a diverse caseload of clients within the DPV catchment areas of Hume and Whittlesea.
  • Contributing to the Paediatric multi-disciplinary team, which may include team-based care plans, case reviews and initiating referrals as appropriate
  • Meeting organisational and funding requirements and KPIs
  • Communicating effectively with clients, colleagues, and other stakeholders
  • Travel across sites will be required. 

About you

  • You will have a Current Registration as a Registered Nurse Division 1 and a current member of APHRA
  • You will have completed an Asthma Educators Course or be willing to complete at the next available course enrolment
  • Clinical Paediatric experience preferred
  • Minimum of 5 years’ experience in the health sector, experience in community health or a community- based setting is desirable.  
  • You will have well-developed clinical skills and demonstrated ability to support a coordinated approach by working with a range of stakeholders to improve health outcomes as agreed with client/family
  • Experience working with clients from CALD backgrounds will be highly regarded.

  • Successful candidates will be required to undergo a National Police Check, hold a valid WWCC and NDIS Worker Screening Check
