About the role

We're looking for a Clinical Case Manager, to provide Home Care Package (HCP) and Short Term Restorative Care (STRC) for older adults, their carers, and families with enhanced access to holistic supports to maintain independent living according to preferences.

Clinical Case Managers at EACH work collaboratively with clients and their support network to monitor and review the individual plan, ensuring it remains responsive to changing needs and circumstances and within an agreed budget plan.

What you'll be doing

This varied role will see you:

  • undertaking initial and ongoing comprehensive assessment, and care planning to support clients at home,
  • monitoring the health and clinical needs of clients, and where needed, providing guidance and decision support,
  • connecting clients with professionals to assist them with occupational therapy needs, home equipment requirements, domestic assistance, or community connections depending on their needs and their plan, and
  • building collaborative relationships with colleagues, local government, service networks, and a range of community agencies in the Taree area.​

About you

To thrive in this role, you will:

  • be clinically experienced with highly developed Assessment and Care Planning skills,
  • have a strong commitment to advocacy and the rights of older adults,
  • be experienced with, and committed to, practice within a person-centred model, and
  • be capable of effectively engaging individuals and families to build positive and sustainable relationships.

Qualifications

  • Current Registered Nurse DIV 1, with APHRA, or equivalent qualified role

Who you'll be working with

Working with other professionals within the regional Older Adults team, you will be one of 6 well-experienced Case Managers in total, supported by our NSW Clinical Lead. The team reports to the NSW Operations Manager who is focused on supporting the team and to giving clients the best possible care.
