North and West Remote Health Ltd (NWRH) are currently seeking a full-time NDIS Support Coordinator to join their passionate team in Mount Isa.

Reporting to the Community Programs and Contract Compliance Manager, you will be responsible for facilitating the implementation of all supports identified in a participant's NDIS plan. You will also be assisting in the development and achievement of a participant's goals to maximise opportunity and independence.

Your day to day responsibilities will include (but will not be limited to):

  • Supporting NDIS participants to build capacity and coordinate their NDIS plans while negotiating appropriate support and services;
  • Coordinating services as defined in the participant's NDIS plan, utilising a capacity-building approach;
  • Preparing, implementing and reviewing service agreements and goal outcomes to ensure it meets the appropriate standards;
  • Resolving crises that impede a participant's ability to achieve their goals; and
  • Developing relationships with key stakeholders and community organisations to grow the demand for Support Coordination services and potentially source new clients

About You

To be considered, you must have at minimum a Cert IV in Disability Support or similar (or significant NDIS sector experience) or lived experience of disability and have demonstrated experience with case management and the provision of support coordination activities.

Your comprehensive knowledge of the NDIS will prove valuable in your ability to locate, coordinate, manage and facilitate a range of relevant supports and support providers to connect participants with them.

As an agile and process-driven Support Coordinator, you will have the skills necessary to work autonomously or as part of a community-based multidisciplinary team to organise structured and comprehensive case management plans. You will also build a strong rapport with participants and develop networks with other service providers throughout the process.

You will also demonstrate exceptional interpersonal skills and communicate with a high level of cultural sensitivity and confidentiality, which will translate through to your record-keeping and data control.

North West Remote Health is ideally looking for a candidate with experience working in rural communities and who demonstrates a commitment to supporting people with disabilities to achieve their goals and has an astute awareness of the complex issues impacting Indigenous communities.

Please note: Due to the nature of this role, you will need a current, unrestricted 'C' class Australian Driver's Licence; be comfortable travelling by road and small planes and staying for one week every month in neighbouring remote communities.

The successful candidate will receive an attractive remuneration of $55,000 – $70,000, commensurate with skills and experience and inclusive of super and salary sacrificing options.

A fantastic range of additional benefits will also be provided, including:

  • Annual leave loading of 17.5%;

  • Relocation assistance and rental subsidies (negotiable).
