About You

With a passion for coaching & development others, you will be motivated by seeing others gain independence in their life. Proven experience of working in Mental Health, Forensics or AOD, supporting clients who demonstrate challenging behaviours as a result of mental health issues or cognitive disability will be matched by the following:

  • Demonstrated ability to coach and lead a team
  • A strong commitment to client-directed care
  • Experience with dual disability or dual diagnosis support
  • Ability to multitask and maintain a positive outlook
  • An understanding of the NDIS and its impact on outcomes for clients and existing models of service delivery
  • Effective listening skills and adaptable communication style
  • Tertiary qualifications in Mental Health, Allied Health, Social Work, Psychology or other relevant qualification

This position requires acquiring and maintaining an NDIS Worker Screening clearance, a positive Working With Children's Check and possess a current & valid driver's licence. Furthermore the health, safety and well being of our staff and clients is of the utmost importance to ermha365. Accordingly, it is highly desirable applicants are fully vaccinated against COVID19.

About the Role

Reporting to the Senior Manager, Mental Health, Forensic & Community Services, this role is responsible for leading high-quality practice for people we support in a new Community based team. This team will be integral in providing wraparound support to complex clients during periods of transition; both out of secure settings into the community & with view to be transitioned into ermha365's Complex Services program where appropriate.

To achieve this the role entails:

  • Team leadership, ensuring staff maintain a focus on client directed needs to achieve client directed outcomes & service delivery objectives
  • Evidenced based best practices, incorporating innovation and creativity for an inclusive model of care
  • Confirming accurate case notes are completed regularly by the team and that regular case reviews are conducted
  • Ensure compliance with all internal & external requirements including reporting
  • Ensuring agreed service delivery is provided to the client
  • Developing and maintaining strong relationships with all relevant stakeholders
  • This role will both be working from home and required to travel across Metro Melbourne and parts of regional Victoria to meet with staff, attend care team meetings and visit clients.
