Help people with a disability achieve their goals and families make the most of their child’s NDIS plan. For many participants and parents, finding their way through the process of planning and connecting with services can be challenging and time consuming. Support Coordinators help people plan, connect to services and overcome barriers.

Extended Families has strong relationships with the Chinese community and our Support Coordination service is very popular with families. To accommodate the growing demand for a Chinese speaking Support Coordinator, we are seeking another worker to join the supportive team.

Who

We are seeking a Mandarin speaking worker (but if you speak Cantonese too that would be great) to complement our culturally sensitive team.You will have:

  • Experience in family case work or case management
  • Bi–lingual Mandarin and English; with good spoken and written English skills
  • Excellent communication, organisation, flexibility and empathy and ability to engage with families
  • A holistic practice orientation and an understanding of the various issues that may impact on a family such as mental health, child protection, relationship and socio-economic issues
  • Strong administration and computer skills
  • Ability to support children and young people with a disability and their families
  • Be culturally sensitive
  • Skills and experience in mental health support (preferred)

We are looking for people with qualifications in Social Work or equivalent.

The successful applicant will have relevant experience in and understanding of Disability Services, a current Victoria Driver’s license and access to a car, and a current satisfactory Police Check and Working with Children Check.

Ideally the position will be full time, but we are also open to a 3 or 4 day a week position.

WHAT

Support Coordinators work with families, carers and NDIS participants to provide independent support, coordinate all aspects of their NDIS plan and to form links with a range of community, mainstream and funded support services. They help participants and their families to implement their NDIS Plan to achieve goals.

This includes assisting people to:

  • Source and assess support options and connect with services
  • Coordinate services
  • Implement, monitor and manage their NDIS plan
  • Build their skills and confidence
  • Respond and deal with any crisis
  • Ensure goals are met, report outcomes to the NDIS and re-develop goals prior to plan review
  • Orientation, training and ongoing support provided.

Where

You will be based at the Extended Families office in Box Hill (and working at home during social isolation), supporting families across Melbourne, primarily in the Eastern Region.

When

Most work is in business hours, although may include occasional after-school or evening visits to connect with relevant family members.
