The position

The Family Practitioner is a Full Time Permanent role based at Sandringham and Frankston and provides short term case management and support service for vulnerable infants, children and young people aged 0-17 and their families.

A day in the life of this position includes:

The Family Practitioner will work with a range of clients presenting with complex needs and issues of risk and safety. Typically the practitioner will conduct weekly outreach visits with their allocated families, provide needs based case management utilising a child-centred strengths-based approach. The aim of the program is to build parenting capacity and assist parents to be responsive to their child’s developmental needs, including their understanding of child development and building healthy and positive attachments. The practitioner will provide a collaborative response to clients’ needs. Working closely with other key stakeholders including Maternal and Child health nurses, Schools, the Department of Families, Fairness and Housing (formerly known as DHHS) and other relevant professionals. The Family Practitioner will work across Case Management in combination with carrying a portfolio which may pertain to relevant group or community based work. Whilst this position will relate predominantly to a case management framework, the incumbent may be required to work across the brief intervention space.
