Position Details

This position is responsible for delivering community based occupational therapy interventions for a diverse range of client to assist eligible clients to remain in their own homes in the community by enhancing informal supports as well as recommending, coordinating, and developing linkages to relevant formal support services.

Key Responsibilities

  • Works independently and exercises independent judgement in the provision Occupation Therapy to community clients.
  • In conjunction with clients/carers undertake a range of holistic assessments of individual client needs, including functional assessments and activities of daily living assessments utilising an occupational therapy framework.
  • Undertake comprehensive health assessments and develop healthcare and support plans with a focus on wellness and restorative practices.
  • Based on the outcome of assessments, provide recommendations to support independence, daily living skills and community engagement.
  • Implement, monitor and regularly review appropriate and relevant individual support plans.
  • Ensure recommendations meet all legal requirements and relevant standards.
  • Assessing and prescribing clinically indicated mobility and daily living aid equipment.
  • Prepare minor home modification recommendations with relevant diagrams and facilitate referral pathways for major home modifications.
  • Complete and submit equipment applications for a variety of government equipment programs.
  • Organise and participate in case conferences, family meetings and interagency meetings in a collaborative and consultative manner.
  • In collaboration with Program Managers, Care Managers and other relevant service providers, support and monitor the appropriateness and responsiveness of support services.
  • Promote Footprints programs in the community through sound working relationships with service provider networks.
  • Ensure that individuals are valued and respected and that support initiatives aim to promote and maintain independence.
  • Assist with program development and administrative functions including maintenance of client data base and funding reporting requirements.
  • Work within the Footprints Clinical Governance Framework.
  • Develop and deliver group education sessions that promote wellness and reablement.
  • Participate in Footprints Work Health and Safety Training Program by providing manual handling training to employees, and participate in equipment (e.g., wheelchairs) maintenance checks as required.
  • Contribute to the ongoing development and evaluation of program practice, ensuring continuous quality improvement and compliance with the Aged Care Quality Standards, the Human Services Quality framework, and the NDIS Practice Standards.
  • Contribute to the supervision of Occupational Therapy students.

Knowledge and Skills - What you'll need

Essential

  • Bachelor of Applied Science in Occupational Therapy and current registration with AHPRA
  • Demonstrated knowledge and experience in community-based assessment and rehabilitation including equipment prescription and home modifications.
  • Demonstrated experience in a Case Management model utilising an Occupational Therapy framework, reflecting a strengths-based approach.
  • Demonstrated knowledge and/or experience in working with a diverse client group including people who are ageing, people with mental illness, dementia and younger people with a disability and their carers/families.
  • Demonstrated knowledge and experience in community networking and working collaboratively with other service providers to plan and provide consistent, responsive, and appropriate client focused support.
  • Well-developed written and verbal communication skills and customer service skills, including a client-centred approach to healthcare and the ability to produce accurate client files.
  • Current Queensland Class C Open driver’s licence
  • NDIS workers screening check or current Yellow Card and Blue Card

Desirable

  • Current First Aid and CPR Certificate
  • Experience working with My Aged Care and NDIS participants
