The Program Leader Healthy Communities provides effective operational management and leadership to staff as they provide a broad range of primary health care, health promotion, therapeutic and community support services, within the Central Grampians and Wimmera sub regions.

Programs within Healthy Communities includes Freeza, Youth Justice, School Focused Youth, Health Promotion, Make a Change, Aboriginal Health, Social Support and Recovery, Nursing (Community Health, Movement Disorder and Take Action).

The position will contribute to achieving the vision, goals and values articulated in the organisation’s strategic plan.

Working from home

Grampians Community Health is progressive with offering the capacity for many of our staff to work remotely from their homes where the service delivery allows for it. Access to information technology is provided to allow you the flexibility of working from home while remaining connected to Grampians Community Health staff, supervision and management.

Professional development

Grampians Community Health values our employees by supporting professional development to develop the knowledge and skills of our staff to better meet the needs of our clients. Clinical supervision is provided for all staff who require it for their professional practice.

THE GRAMPIANS WIMMERA REGION

The Grampians Wimmera region is renowned for its friendly welcoming community, affordable housing, close access to regional centres and the best of Victoria’s natural environment on its doorstep. More than a ‘tree change’, the Grampians Wimmera is a lifestyle like no other.

Stawell is the closest large town to the Grampians National Park and Halls Gap. It is the home of the Stawell Gift and operates one of Victoria’s largest gold mines.

Ararat is a former gold mining town within a wine and fruit growing district. It is the home of the Gum San Chinese Heritage Centre, the Langi Morgala Museum and the famous J Ward and Aradale.

Horsham is known as the capital of the Wimmera, located on the banks of the Wimmera River with easy access to a dynamic arts scene and legendary natural attractions such as Mount Arapiles, the Grampians and the Little Desert.

All Grampians Community Health employees are required to hold a:

  • Current National Police Check (no more than 3 months old from date of issue)
  • Working With Children Check for child related work

And provide proof of qualifications before commencement.
