Clinical Care Coordinator

ABout the role

The Clinical Care Coordinator will work 3 days a week, 10am to 3pm and will provide leadership to the team, ensuring a high quality of care and service delivery. You will be an experienced, dedicated, motivated and enthusiastic Registered Nurse with a passion for the provision of high quality, innovative care and services to our Aged Care Consumers .

You will support each consumer to achieve their individual and health goals through clinical and holistic case management, in partnership with their families, medical practitioner(s), allied health and our clinical care team.

  • Managing a caseload of clients ranging from high functioning, low risk through to complex care needs.
  • Develop care plans within Home Care Package guidelines for clients
  • Ensuring that the organisation's customer service expectations are exceeded in the administration.
  • Complete documentation to ensure compliance with policies and procedures.
  • Liaise with General Practitioners, Allied Health Professionals, community organisations and internal clinical care providers to ensure client needs are met and well-coordinated.
  • Representing Gold Star as a service provider and providing information on our service provisions
  • Connecting people with appropriate services and supports in their community

Key selection criteria 

  • Max of 2 years + experience in case management, assessment or planning in the Aged, Community, Health or Social Services
  • Qualifications and current registration in Nursing or Allied Health (highly desired)
  • Current Senior First Aid and CPR
  • Current and unrestricted driver’s license.
  • Understanding of the Aged Care Sector and Community Program guidelines and service requirements. In particular, Home Care Package Guidelines (HCP)
  • Excellent interpersonal, rapport building and communication skills (listening, written and verbal).
  • Experience working remotely with a high degree of drive
  • Ability to network, develop and manage relationships with internal and external stakeholders
  • Excellent time management skills
  • Ability to work in a fast-paced, innovative and ever-changing organisation
